A 55-year-old man was admitted to a tertiary referral center with severe right-sided weakness and a Glasgow Coma Score of 8. The onset of symptoms was estimated to be 6 hours prior to admission. The patient exhibited bilaterally up-going plantar responses and had no major co-morbidities. A CT scan of the brain revealed a significant area of infarction within the territory of the left middle cerebral artery, accompanied by massive cerebral edema and midline shift.
