Multiple sclerosis is an uncommon diagnosis in late-middle-aged men and is an upper motor neurone disease, whereas this patient has a combination of upper and lower motor neurone signs. There is no evidence of tremor or bradykinesia in this case.

Understanding Motor Neuron Disease: Signs and Diagnosis

If there is a color change, it indicates the presence of H. pylori urease activity. Serum antibody tests can also be used, but they remain positive even after eradication. Culture of gastric biopsy can provide information on antibiotic sensitivity, while histological evaluation alone can be done through gastric biopsy. Lastly, the stool antigen test has a sensitivity of 90% and specificity of 95%.

There is no need to take any action regarding his cholesterol as it is under control. Additionally, his blood pressure is also well managed. However, his HbA1c level is in the pre-diabetes range (42-47 mmol/mol) and requires further investigation. It is important to note that a HbA1c reading alone cannot rule out diabetes, and a fasting sample should be arranged for confirmation.

Type 2 diabetes mellitus can be diagnosed through a plasma glucose or HbA1c sample. The diagnostic criteria vary depending on whether the patient is experiencing symptoms or not. If the patient is symptomatic, a fasting glucose level of 7.0 mmol/l or higher or a random glucose level of 11.1 mmol/l or higher (or after a 75g oral glucose tolerance test) indicates diabetes. Only patients with a 10-year cardiovascular risk > 10% should be offered a statin, with atorvastatin 20mg as the first-line choice.

Obesity in children can lead to various consequences, including orthopaedic problems such as slipped upper femoral epiphyses and Blount’s disease, musculoskeletal pains, psychological consequences like poor self-esteem and bullying, sleep apnoea, and benign intracranial hypertension. Moreover, obesity in childhood can increase the risk of developing type 2 diabetes mellitus, hypertension, and ischaemic heart disease in the long run. Therefore, it is crucial to identify and manage obesity in children to prevent these adverse outcomes.

Stephen’s symptoms of progressive peripheral polyneuropathy and hyporeflexia strongly suggest Guillain-Barre syndrome, likely triggered by a recent gastrointestinal infection. Myasthenia gravis, on the other hand, presents with muscle fatigue and ocular manifestations, but normal tone, sensation, and reflexes. Polymyositis causes proximal muscle weakness, while acute transverse myelitis presents with paralysis of both legs, sensory loss, and bladder/bowel dysfunction. Other features of Guillain-Barre syndrome may include a history of gastroenteritis, respiratory muscle weakness, cranial nerve involvement, diplopia, bilateral facial nerve palsy, oropharyngeal weakness, and autonomic involvement. Autonomic involvement may manifest as urinary retention or diarrhea. Less common findings may include papilloedema, which is thought to be secondary to reduced CSF resorption.

However, there are several treatment options available, including nonsteroidal anti-inflammatory drugs (NSAIDs), physiotherapy, oral corticosteroids, and intra-articular corticosteroids.

While some patients may experience a burning sensation when consuming hot or spicy foods, most are asymptomatic. The diagnosis of geographic tongue can usually be confirmed from history and physical examination alone, and no medical intervention is required.

Surgical correction is not warranted for geographic tongue, as it is a benign condition of no clinical significance. Aspiration for cytology is also not appropriate, as the diagnosis can be made without further investigation. Excision biopsy may actually cause harm, as histological findings are identical to those seen in pustular psoriasis.

A full blood count is not necessary for the diagnosis of geographic tongue. Instead, the condition can be identified through the elongation of rete ridges, hyperparakeratosis and acanthosis at the periphery, loss of filiform papillae, and migration and clustering of neutrophils within the epithelium towards the centre of the lesions. The predominant inflammatory infiltrates in the lamina propria is neutrophils with an admixture of chronic inflammatory cells.

Orbital cellulitis is a serious infection that affects the fat and muscles behind the orbital septum within the orbit, but not the globe. It is commonly caused by upper respiratory tract infections that spread from the sinuses and can lead to a high mortality rate. On the other hand, periorbital cellulitis is a less severe infection that occurs in the superficial tissues anterior to the orbital septum. However, it can progress to orbital cellulitis if left untreated.

Risk factors for orbital cellulitis include childhood, previous sinus infections, lack of Haemophilus influenzae type b (Hib) vaccination, recent eyelid infections or insect bites, and ear or facial infections. Symptoms of orbital cellulitis include redness and swelling around the eye, severe ocular pain, visual disturbance, proptosis, ophthalmoplegia, eyelid edema, and ptosis. In rare cases, meningeal involvement can cause drowsiness, nausea, and vomiting.

To differentiate between orbital and preseptal cellulitis, doctors look for reduced visual acuity, proptosis, and ophthalmoplegia, which are not consistent with preseptal cellulitis. Full blood count and clinical examination involving complete ophthalmological assessment are necessary to determine the severity of the infection. CT with contrast can also help identify inflammation of the orbital tissues deep to the septum and sinusitis. Blood culture and microbiological swab are also necessary to determine the organism causing the infection.

The recommended treatment for ascites in this patient with liver cirrhosis is spironolactone. It is used to treat various conditions such as ascites, hypertension, heart failure, nephrotic syndrome, and Conn’s syndrome. In patients with cirrhosis, spironolactone is often prescribed in relatively large doses of 100 or 200mg to counteract secondary hyperaldosteronism. It is also used as a NICE ‘step 4’ treatment for hypertension. However, it is important to note that spironolactone can cause hyperkalaemia and gynaecomastia, although the latter is less common with eplerenone.

One study, known as RALES, found that low dose spironolactone can reduce all-cause mortality in patients with NYHA III + IV heart failure who are already taking an ACE inhibitor. It is important to consult with a healthcare professional before taking spironolactone to determine if it is the right medication for your specific condition and to monitor for any potential adverse effects.

Investigations for Infective Endocarditis: Choosing the Most Appropriate Initial Test

When a patient presents with fever and a new murmur, infective endocarditis is a likely diagnosis until proven otherwise. The most appropriate initial investigation is a blood culture, with three samples taken from different sites before starting antibiotics. Positive blood cultures are a major criterion for diagnosing infective endocarditis.

While other investigations may be useful in aiding diagnosis, they are not as specific as blood cultures. An ECG may show evidence of an aortic root abscess and its possible sequelae, such as AV block. A 24-hour ECG is helpful in diagnosing paroxysmal arrhythmias.

C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) are non-specific markers that may be raised in infective endocarditis, but also in other conditions such as infection, malignancy, and pregnancy.

In summary, when suspecting infective endocarditis, the most appropriate initial investigation is a blood culture. Other investigations may be useful in aiding diagnosis, but are not as specific as blood cultures.

The most common adverse effect associated with the progesterone-only pill, also known as the ‘mini pill’, is irregular vaginal bleeding. Patients should be advised to take the pill at the same time every day without a pill-free break. While oily skin may occur due to increased sebum production, this is not as common as irregular vaginal bleeding. Patients with a history of acne vulgaris who take the progesterone-only pill may experience increased sebum production. The combined oral contraceptive pill (COCP) is commonly used to manage acne vulgaris and hirsutism in polycystic ovarian syndrome. Headaches may occur during the first few months of using hormonal contraception, but this is less likely to occur with the POP as the most common time to experience headaches is during the ‘pill-free interval’. Patients with a history of migraine with aura should be advised of the risks associated with the COCP and migraine with aura and informed that the POP is likely to be a safer alternative. While mood changes have been studied with birth control, there is no definite outcome of recent studies, and epidemiological data gathered by NICE does not support this as the most common adverse effect.

Counselling for Women Considering the Progestogen-Only Pill

Women who are considering taking the progestogen-only pill (POP) should receive counselling on various aspects of the medication. One of the most common potential adverse effects is irregular vaginal bleeding. When starting the POP, immediate protection is provided if it is commenced up to and including day 5 of the cycle. If it is started later, additional contraceptive methods such as condoms should be used for the first 2 days. If switching from a combined oral contraceptive (COC), immediate protection is provided if the POP is continued directly from the end of a pill packet.

It is important to take the POP at the same time every day, without a pill-free break, unlike the COC. If a pill is missed by less than 3 hours, it should be taken as normal. If it is missed by more than 3 hours, the missed pill should be taken as soon as possible, and extra precautions such as condoms should be used until pill taking has been re-established for 48 hours. Diarrhoea and vomiting do not affect the POP, but assuming pills have been missed and following the above guidelines is recommended. Antibiotics have no effect on the POP, unless they alter the P450 enzyme system, such as rifampicin. Liver enzyme inducers may reduce the effectiveness of the POP.

In addition to these specific guidelines, women should also have a discussion on sexually transmitted infections (STIs) when considering the POP. It is important for women to receive comprehensive counselling on the POP to ensure they are aware of its potential effects and how to use it effectively.

To prevent future torsion of the other testicle, it is recommended that both testes be fixed during testicular torsion surgery. While manual rotation may be successful in some cases, it is not appropriate once surgery has begun. If a bell clapper deformity is present, both testes should be fixed if they are still viable. Given the short duration of symptoms (only six hours), it is unlikely that surgical removal (orchiectomy) will be necessary.

The treatment for testicular torsion is urgent surgical exploration. If a torted testis is identified, both testes should be fixed, as the condition of bell clapper testis is often bilateral.

Adenomyosis is the condition where the tissue lining the uterus (endometrium) grows into the muscular wall of the uterus (myometrium).

Dysmenorrhoea is a condition where women experience excessive pain during their menstrual period. There are two types of dysmenorrhoea: primary and secondary. Primary dysmenorrhoea affects up to 50% of menstruating women and is not caused by any underlying pelvic pathology. It usually appears within 1-2 years of the menarche and is thought to be partially caused by excessive endometrial prostaglandin production. Symptoms include suprapubic cramping pains that may radiate to the back or down the thigh, and pain typically starts just before or within a few hours of the period starting. NSAIDs such as mefenamic acid and ibuprofen are effective in up to 80% of women, and combined oral contraceptive pills are used second line for management.

Secondary dysmenorrhoea, on the other hand, typically develops many years after the menarche and is caused by an underlying pathology. The pain usually starts 3-4 days before the onset of the period. Causes of secondary dysmenorrhoea include endometriosis, adenomyosis, pelvic inflammatory disease, intrauterine devices, and fibroids. Clinical Knowledge Summaries recommend referring all patients with secondary dysmenorrhoea to gynaecology for investigation. It is important to note that the intrauterine system (Mirena) may help dysmenorrhoea, but this only applies to normal copper coils.

There are several common gastrointestinal disorders that can affect newborns. These include pyloric stenosis, necrotising enterocolitis (NEC), congenital duodenal atresia, Hirschsprung’s disease, and tracheoesophageal fistula (TOF).

Pyloric stenosis is characterised by hypertrophy of the circular pyloric muscle, and typically presents with non-bilious, projectile vomiting in the third or fourth week of life. Constipation and dehydration may also occur, and biochemistry may show hypokalaemic metabolic alkalosis. Boys are more likely to be affected, especially if born into a family with affected girls.

NEC is a condition primarily seen in premature infants, where portions of the bowel undergo necrosis. Symptoms include bilious vomiting, distended abdomen, and bloody stools, with late signs including bowel perforation and multi-organ failure.

Congenital duodenal atresia is a congenital absence or complete closure of a portion of the lumen of the duodenum, and presents with bile-stained vomiting, abdominal distension, and inability to pass meconium.

Hirschsprung’s disease is characterised by the failure of ganglion cells to migrate into the hindgut, leading to functional intestinal obstruction. Symptoms include abdominal distension, bile-stained vomiting, and failure to pass meconium.

Postmenopausal bleeding is the most common symptom of endometrial cancer, which is usually slight and intermittent initially before becoming more heavy. Pain is not common and typically signifies extensive disease, while vaginal discharge is unusual.

Immunofluorescence reveals intra-epidermal circular deposits, and antibodies against desmosomes are usually positive. The condition requires aggressive treatment, with high-dose steroids initially used, and life-long maintenance doses often needed. Immunologicals are being trialled.

Bullous Pemphigoid: Another Autoimmune Blistering Condition

Bullous pemphigoid is another autoimmune blistering condition that typically affects older patients. The condition is caused by antibodies directed to the deeper basement membrane of the skin. This results in tense, firm, often haemorrhagic blisters that do not rupture easily. Oral lesions are rare, and Nikolsky’s sign is negative. Immunofluorescence reveals linear IgG and C3 deposits along the basement membrane of the skin, and antibodies against hemidesmosomes may be positive. The condition carries significantly less morbidity than pemphigus vulgaris, and topical steroid cream (ex: clobetasol propionate usually suffices).

If the patient’s condition worsens, ITU may be consulted. However, before referring the patient, it is important to determine if she has already received basic sepsis care. Although studies have explored the use of steroids in sepsis, they are not presently recommended due to an increase in mortality.

In recent years, the classification of sepsis has changed, with the old category of severe sepsis no longer in use. The term ‘systemic inflammatory response syndrome (SIRS)’ has also fallen out of favor.

To manage sepsis, it is important to identify and treat the underlying cause of the infection and support the patient regardless of the cause or severity. However, if any red flags are present, the ‘sepsis six’ should be started immediately. This includes administering oxygen, taking blood cultures, giving broad-spectrum antibiotics, giving intravenous fluid challenges, measuring serum lactate, and measuring accurate hourly urine output.

NICE released its own guidelines in 2016, which focus on the risk stratification and management of patients with suspected sepsis. For risk stratification, NICE recommends using red flag and amber flag criteria. If any red flags are present, the sepsis six should be started immediately. If any amber flags are present, the patient should be closely monitored and managed accordingly.

To help identify and categorize patients, the Sequential (Sepsis-Related) Organ Failure Assessment Score (SOFA) is increasingly used. The score grades abnormality by organ system and accounts for clinical interventions. A SOFA score of 2 or more reflects an overall mortality risk of approximately 10% in a general hospital population with suspected infection. Even patients presenting with modest dysfunction can deteriorate further, emphasizing the seriousness of this condition and the need for prompt and appropriate intervention.

Differentiating Leukaemia and Lymphoma: Characteristics and Symptoms

Leukaemia and lymphoma are both types of blood cancer, but they have distinct characteristics and symptoms. In this context, we will discuss the characteristics and symptoms of chronic lymphocytic leukaemia (CLL), acute lymphoblastic leukaemia (ALL), chronic myeloid leukaemia (CML), Hodgkin’s lymphoma (HL), and multiple myeloma (MM).

CLL is the most common leukaemia in adults, characterized by peripheral blood lymphocytosis and uncontrolled proliferation of B cell lymphocytes in the bone marrow, lymph nodes, and splenomegaly. Patients are often asymptomatic, and CLL is often picked up incidentally.

ALL is a common leukaemia of children aged 2–5 years and is very rare in adults.

CML tends to present with more systemic, B symptoms in a slightly younger age group. A classic symptom is a massive hepatosplenomegaly. Blood film would show PMNs, basophils, and myelocytes.

HL is a possibility in this age group; however, the question asks for the ‘most likely’ diagnosis, which would be CLL as it is more common in this age group and the western world. Further investigation would be used to confirm the diagnosis.

MM is the uncontrolled proliferation of plasma cells and does not present in the way described above. Bone pain, hypercalcaemia, renal failure, and neutropenia are common presentations.

Management of Acute Asthma

Acute asthma is classified by the British Thoracic Society (BTS) into three categories: moderate, severe, and life-threatening. Patients with any of the life-threatening features should be treated as having a life-threatening attack. A fourth category, Near-fatal asthma, is also recognized. A chest x-ray is not routinely recommended unless the patient has life-threatening asthma, suspected pneumothorax, or failure to respond to treatment.

Patients who fail to respond require senior critical care support and should be treated in an appropriate ITU/HDU setting. Criteria for discharge include stability on discharge medication, checked and recorded inhaler technique, and PEF levels above 75% of best or predicted.

Crohn’s Disease: This inflammatory bowel disease can affect any part of the gastrointestinal (GI) tract and can also cause extraintestinal manifestations such as arthritis, iritis, and rashes. However, nasal polyps are not associated with this condition.

Diabetes Mellitus: While nasal polyps are not associated with diabetes mellitus, other conditions including coeliac disease, polycystic ovarian syndrome, and thyroid dysfunction can all develop in affected individuals.

Hereditary Spherocytosis: This condition is characterized by abnormally shaped erythrocytes and symptoms such as fatigue and shortness of breath. Clinical signs include pallor, jaundice, and splenomegaly. However, nasal polyps are not associated with this condition.
