Type II hypersensitivity occurs when cell-bound IgG or IgM binds to an antigen on the cell surface, leading to autoimmune conditions such as autoimmune hemolytic anemia, ITP, and Goodpasture’s syndrome. Type III hypersensitivity occurs when free antigen and antibody (IgG, IgA) combine to form immune complexes, leading to conditions such as serum sickness, systemic lupus erythematosus, and post-streptococcal glomerulonephritis. Type IV hypersensitivity is T-cell mediated and includes conditions such as tuberculosis, graft versus host disease, and allergic contact dermatitis.
In recent times, a fifth category has been added to the classification of hypersensitivity reactions. Type V hypersensitivity occurs when antibodies recognize and bind to cell surface receptors, either stimulating them or blocking ligand binding. This type of reaction is seen in conditions such as Graves’ disease and myasthenia gravis. Understanding the classification of hypersensitivity reactions is important in the diagnosis and management of these conditions.
