A 75-year-old man presents to the emergency department after collapsing. He has a medical history of hypertension and atrial fibrillation and takes amlodipine and apixaban. A recent 24-hour tape showed sustained atrial fibrillation with episodes of bradycardia (minimum heart rate 20 beats per minute) and symptoms of presyncope. The admission ECG shows slow atrial fibrillation with a heart rate of 24 beats per minute. The patient has since recovered, with a Glasgow coma scale of 15/15 and a heart rate of 82 beats per minute.
