He is currently taking ramipril, metformin, atorvastatin, and gliclazide. On examination, his blood pressure is 129/72 mmHg, and his pulse is 81. Bibasal crackles are heard on auscultation of his chest.

– Glucose: 12.3 mmol/L (<7.0)
– ECG: Anterolateral ST depression
